「キツツキの穴が空いてる」is not an idiom, it's probably a metaphor, meaning 'revealing the truth'. Sometimes「ように」is just ommited, you know.

Uhm, it turns out this expression means 'to have no brains'. So the sentence "Because this would open a woodpecker’s hole in the faces of the citizens? Of everyone?" should be translated as "Does everyone in this country has a hole in their head?" or something like that. Hmm... Then I take it to mean that the citizens have no brains inside their skulls? (Hence a 'hole' in their heads). In that case, I would translate it as 'Does everyone in this country have empty heads?' or 'empty skulls'.


Well, yeah, Shizu is just wondering if everyone in this country is that stupid ('being unable to appreciate such a beautiful art').
